Illness Exclusion Notice – (Updated Dec 2025) When you receive a call from ACCC to pick up a sick child, ACCC will also give you an illness exclusion notice and will let you know when your child can return to school. Please sign this notice when you recieve it.
LIC 9221 – Parent Consent for Administration of Medications – The parent shall provide all medication to be administered to the child. Regardless of how the medication is administered, a release chart must be filled out and the medications must be stored in the original box with the unaltered label.
